Application Process

Who Is Eligible?

  • Residents of Louisville/Jefferson County
  • Children who will be 4 years old on or before August 1 who meet family income guidelines
  • Children who will be 4 years old on or before August 1 and have a diagnosed disability
  • Children who will be 3 years old on or before August 1 and have a diagnosed disability

How to Apply

  • If you cannot apply online, you may call (502) 485-7677 to schedule an appointment for a phone application with a member of the Early Childhood Student Assignment team.

Required Documents

  • Picture ID required for parent or legal guardian
  • Custodial papers (if applicable)
  • Proof of residence (includes parent/guardian name and address)
    • Gas/Electric or water bill (within past three months)
    • Lease/House contract
    • Paycheck or paycheck stub
    • Social Security or other government check with address shown
  • Child’s proof of birth
    • Official state-issued birth certificate (Any other proof of birth documentation must be state-approved and be accompanied by a signed affidavit.)
    • Federal travel document
  • Family income verification
    • Previous year’s tax return or all household W2s and 1099s
    • Unemployment benefits statement/SSI/Any other documents of financial support
    • Child support for all children
