Direct Deposit Form: Direct deposit is mandatory for all JCPS employees. Submit this form to the Payroll Department for new enrollments or changes to your direct deposit information. After the form has been received by the Payroll Department, allow a minimum of three weeks for direct deposit change requests to take effect. Direct deposit changes for summer escrow checks must be received by June 1. Confirmation letters of direct deposit enrollment or changes are mailed to the employee.

Kentucky Certificate of Nonresidence: Complete this form and submit it to the Payroll Department only if you are a resident of Indiana and do not wish to have Kentucky income tax withheld from your paychecks.
