Jefferson County Public Schools (JCPS) is committed to ensuring that every student and new staff member is healthy, safe, and fully prepared for the upcoming school year. To support our community, the district is hosting a series of free walk-in physical and immunization clinics at convenient locations across the city.
No appointments are necessary for these main clinic dates. Families and new staff members can report to any of the designated sites listed below.
Please note: Completed parental consent forms are strictly required for both physical examinations and vaccinations before any medical services can be provided to minors. If you need copies of the consent forms sent to you, please contact the Health & Wellness Office at 502-485-3387.

For families and new staff members who cannot attend the scheduled walk-in clinics, JCPS offers alternative options by appointment. You can schedule a visit at one of our dedicated health locations by calling the JCPS Health and Wellness Office at (502) 485-3387.
Appointment-based services are available from 8:30 AM to 4:00 PM daily at the following sites:
South Central Neighborhood Place: 4255 Hazelwood Avenue
CB Young Service Center: 3001 Crittenden Drive
