Form 1095-C

JCPS employees will receive Form 1095-C as required by the Affordable Care Act (ACA). This form will report whether you were offered health insurance coverage, if the coverage was affordable according to ACA standards, and additional details about the health insurance coverage offered to employees. The 1095-C form will supplement but not replace your W-2. Please consult your tax professional for specific questions regarding how Form 1095-C will be used when filing your income tax return.
